Efficacy and Safety of Hybrid Cardiac Telerehabilitation in Patients with Hypertrophic Cardiomyopathy without Left Ventricular Outflow Tract Obstruction and Preserved Ejection Fraction—A Study Design

چکیده
Hypertrophic cardiomyopathy (HCM) is the most common congenital disease increasing risk of sudden cardiac death. For many years, HCM patients were excluded from exercise training. However, there are data showing that with undergoing supervised training could improve physical performance without serious adverse events. A project was designed as a randomized clinical trial to assess effectiveness and safety hybrid rehabilitation (HCR)—a combination hospital-based (1 month) new form home-based telemonitored (2 months) in left ventricular (LV) outflow tract obstruction preserved systolic function. Sixty who fulfil inclusion criteria have been randomly assigned (1:1) either HCR plus usual care (training group) or only (control group). The primary endpoint functional capacity evaluated by peak oxygen uptake (pVO2). Secondary endpoints include workload time during cardiopulmonary testing, six-minute walk test distance, NT-pro BNP level, echocardiographic parameters diastolic function (E/A, E/e’, myocardial strain rate), right pressure, gradient LV tract, quality life. tertiary analysis includes safety, acceptance adherence program. Our research will provide innovative on Clinical trials registry: ClinicalTrials.gov Identifier NCT03178357.

Left ventricular outflow tract obstruction and sudden death risk in patients with hypertrophic cardiomyopathy.
AIMS Left ventricular outflow tract obstruction (LVOTO) is associated with reduced survival in patients with hypertrophic cardiomyopathy (HCM). The influence of LVOTO on survival from SD in relation to other recognized clinical risk markers is unknown. METHODS AND RESULTS A total of 917 patients with HCM (554 males, 43+/-15 years) were studied; 288 (31.4%) had LVOTO at rest (> or =30 mmHg). D...
متن کاملEvaluation of left ventricular outflow tract obstruction with 4D phase contrast in patients with hypertrophic cardiomyopathy
Background Patients with hypertrophic cardiomyopathy (HCM) develop left ventricular outflow tract (LVOT) obstruction due to combination of asymmetric septal hypertrophy and systolic anterior motion of the mitral leaflet. Degree of LVOT obstruction is an important predictor in patient symptoms and clinical outcome. LVOT obstruction is currently assessed by Doppler echocardiography (echo) using p...
متن کاملAnomalous Papillary Muscle Insertion Causing Dynamic Left Ventricular Outflow Tract Obstruction without Hypertrophic Obstructive Cardiomyopathy
Anomalous papillary muscle insertion directly into the surface of the mitral valve leaflet is rare, especially in a subject without apparent evidence of hypertrophic cardiomyopathy. We present a case of this isolated congenital malformation producing two hemodynamic sequelae of dynamic left ventricular outflow tract obstruction and severe mitral regurgitation.
متن کاملDiverse geometric changes related to dynamic left ventricular outflow tract obstruction without overt hypertrophic cardiomyopathy
BACKGROUND Dynamic left ventricular (LV) outflow tract (LVOT) obstruction (DLVOTO) is not infrequently observed in older individuals without overt hypertrophic cardiomyopathy (HCM). We sought to investigate associated geometric changes and then evaluate their clinical characteristics. METHODS A total of 168 patients with DLVOTO, which was defined as a trans-LVOT peak pressure gradient (PG) hi...
متن کاملVariability of left ventricular outflow tract gradient during cardiac catheterization in patients with hypertrophic cardiomyopathy.
OBJECTIVES This study characterizes left ventricular outflow tract (LVOT) gradient variability in patients with hypertrophic cardiomyopathy (HCM) during cardiac catheterization. BACKGROUND Management of HCM is directed by the presence and magnitude of LVOT obstruction. The magnitude and clinical impact of spontaneous variability during a single cardiac catheterization has not been described. ...
